As a monoamine oxidase inhibitor (MAOI), it can increase the risk of serotonin syndrome when taken with other serotonergic drugs such as SSRIs, SNRIs, and tricyclic antidepressants. This condition is characterized by symptoms like agitation, confusion, rapid heart rate, and high blood pressure, and in extreme cases, can be fatal.
Additionally, methylene blue can interact with medications like acetazolamide, antacids, sodium bicarbonate, diuretics, and blood thinners, potentially altering their effectiveness or increasing side effects. Even "safe" and common supplements like St. John's Wort, 5-HTP, and CoQ10 can have adverse reactions with Methylene Blue.
The Herxheimer reaction is an immune response that occurs when large quantities of toxins are released into the body as bacteria or other pathogens die off during antimicrobial treatment. This reaction is characterized by symptoms such as fever, chills, headache, muscle pain, diarrhea, fatigue, and worsening of existing symptoms. It typically occurs within hours to a few days after starting treatment and is often seen in conditions like Lyme disease, syphilis, and other infections treated with antibiotics or antimicrobials.
When using an antimicrobial like methylene blue, which has potent antibacterial properties, the rapid killing of bacteria can lead to a significant release of endotoxins into the bloodstream. This sudden influx of toxins triggers an acute inflammatory response from the immune system, resulting in the Herxheimer reaction. While this reaction can be uncomfortable and alarming, it is generally a sign that the treatment is effectively targeting the infection.
